American Family Care Founder Invests in Chronic Care Start-Up

Bruce Irwin, MD, Founder and Chairman of American Family Care, has made a significant investment in Pack Health, a chronic disease management company that provides patients with personalized support. The investment will accelerate the further development of Pack Health's health management platform, paving the way for its aggressive growth plans. Irwin will join the company's board of directors.

Pack Health's engagement programs are designed for patients with chronic ailments such as type 2 diabetes, cancer, COPD and other health-related issues, and address behaviors that impact their health conditions. At the core of Pack Health's programs are Health Advisors, who provide customized support that enables individuals to take control of specific conditions and navigate the healthcare system.

According to the CDC, about half of US adults face at least one chronic illness and one in four Americans have multiple chronic conditions that last a year or more and require ongoing medical attention.
